Forgotten User Name Retrieval

To retrieve your user name complete the form below and press the "Retrieve User Name" button.

Enter your First Name and Last Name and press the "Retrieve User Name" button.

This will display your User Name. In instances where there are multiple users with the same name, the district name is also displayed. This will allow you to select the correct user.
